Strawberry milk: the calcium of milk with sugar added

In a meta-analysis of randomized trials in children, replacing sugar-sweetened drinks with either zero-calorie drinks or flavored milk lowered body fat by 0.70 percentage points (95% CI 0.62 to 0.78), in only three trials that pooled milk with the zero-calorie drinks. The same swap did not change BMI z-score in two trials (minus 0.05, 95% CI minus 0.20 to 0.09). In a meta-analysis of controlled feeding studies in people with and without diabetes, sweetened low fat milk eaten in place of other energy for at least a week raised fasting insulin by about 19 pmol/L (95% CI 9 to 29). That result rests on two study comparisons of sweetened milk in general, in a review whose evidence was rated low overall. A 248 g cup, the size of a US school carton, gives about 161 calories, 21 g of total sugars, 300 mg calcium and 7.9 g protein, and the sugar figure counts the lactose that plain milk already has along with the added sugar. The whole milk version comes to about 213 calories and 29 g sugars.

The bottom line

In the UK table for strawberry or banana flavored milk, 3.9 g of the 8.9 g sugars per 3.5 oz (100 g) is sucrose and 4.9 g is lactose, and that record does not separate the two flavors. A review of 13 mostly non-randomized, high-bias studies in children, over half funded by industry or undisclosed, found flavored milk raised total milk intake in 11 and energy intake in 5 of the 7 that checked. Weight gain rose in 1 of the 3 that looked. Observational US survey data from single-day food recalls link flavored milk in girls aged 12 to 18 with calcium intake of 992 mg a day, close to the 1,038 mg of plain milk drinkers and far above the 576 mg of girls who drank none. In that survey, which had non-government funding, flavored milk drinkers took in no more added sugar than children who drank no milk. A review of US school lunch studies says removing flavored milk from schools may lower how much milk children drink, and it gives a direction with no size. In 69 women with obesity on a six-month weight loss diet, milk with 15 percent sucrose came to be liked more than the 30 percent version they had liked equally at the start, a change over time measured on taste alone. We have no evidence card on lactose intolerance, milk allergy or medicines for strawberry milk, so the page has no safety section, and that is a gap in what we found.

Of 3978 studies identified, 13 met inclusion criteria. Ten studies were experimental and three were longitudinal cohort studies. Eleven studies found that flavored milk increased overall milk intake, five of seven studies that examined energy intake showed that flavored milk increased energy intake, and one of three studies that assessed obesity outcomes demonstrated an increase in weight gain with flavored milk consumption. Only one study was a randomized controlled trial, most studies had high bias, and over half were industry-funded or did not disclose funding.
