Huevos rancheros: what the eggs and the tortilla each bring

Nobody has studied huevos rancheros as a dish, so everything here comes from its parts, eggs and lime-cooked corn tortillas. The strongest egg result comes from feeding trials in young children, not from this dish. Across 7 randomized trials with 3673 children aged 6 to 59 months in sub-Saharan Africa, egg-based feeding raised weight-for-age z-score by 0.33 (95% CI 0.11 to 0.55) and made no significant difference to height-for-age. For adults the question is usually cholesterol. An umbrella review of 14 meta-analyses found eggs linked with higher LDL cholesterol, with a pooled difference of 7.39 in units the abstract does not state, and it rated every significant association as very weak evidence. The same review found no association between high and low egg intake and any cardiovascular outcome or death. A Nordic review of 38 systematic reviews reached a similar place: eggs can push blood cholesterol up in trials, and one egg a day is unlikely to change overall disease risk.

The bottom line

A US survey serving is 118 g per egg, and an unspecified plate is coded at 236 g. USDA has no nutrient record for the dish, so we cannot give calories or protein for it. The tortilla has its own story. Cooking maize with lime, called nixtamalization, adds calcium, and phytate in the corn holds much of it back because the process removes only part of the phytate. In an isotope study of just 5 women eating about 4.9 oz (140 g) of tortillas, they absorbed 50 percent of the calcium from a special low-phytate maize against 35 percent from ordinary maize. In a randomized trial with 40 adults in Soweto who ate deliberately moldy maize, nixtamalization cut the mold toxin fumonisin B1 in the grain by 95 percent and zearalenone by 89 percent, measured in the maize and in urine rather than as illness. Egg allergy is the caution that matters here. In a single-center review of 123 egg-allergic children with a median age of 12 months, 115 tolerated boiled egg yolk in a supervised challenge, and huevos rancheros uses fried whole egg, which that study did not test. A background statement in another study puts tolerance of baked forms at about three quarters of children with egg or milk allergy, which also says nothing about a fried egg. In adults on dialysis, egg white given as a supplement raised serum albumin by 0.42 g/dL in 4 trials rated very low certainty, and lowered phosphorus by 2.04 mg/dL in 2 trials rated low certainty. That is egg white, prescribed, so it does not tell a dialysis patient to eat whole-egg dishes. We found no card on this dish in pregnancy or with medicines.

At 1-mo intervals, 5 healthy adult women were fed 2 test meals of approximately 140 g tortillas in lieu of breakfast. ... Mean fractional absorption of calcium from tortillas prepared from the low-phytate maize (0.50 +/- 0.03) was significantly (P = 0.003) greater than that from tortillas prepared from the control maize (0.35 +/- 0.07).

Calcium fortification of maize has been achieved for millennia in Central America by the process of nixtamalization. Bioavailability of calcium is, however, compromised by phytate, which is present in large quantities in maize kernels and is only modestly reduced by nixtamalization.

Forty adult healthy volunteers from an informal settlement in Kliptown, Soweto (South Africa), were randomly assigned to consume control maize or visibly moldy maize subjected to nixtamalization. Nixtamalization achieved a reduction in fumonisin B3 and deoxynivalenol (DON) to unquantifiable or undetectable levels in maize, while reducing fumonisin B1 (FB1), fumonisin B2, and zearalenone (ZEN) by 95%, 95%, and 89%, respectively.
