Can you get too much phosphorus?

What this means for you

For healthy kidneys, phosphorus is self-regulating. The ceiling is 4,000 mg a day for adults up to 50, and the kidneys clear whatever arrives above what the body needs. Very high intakes over a short period can push blood phosphorus up, and in practice that takes supplements or medical situations rather than meals.

The phosphorus question that actually matters is about kidneys rather than about dose.

Not for everyone. In chronic kidney disease the clearance mechanism fails, phosphorus accumulates, and the whole page inverts. Targets there come from your own clinician and they are personal. Even so, the evidence behind the treatment is weaker than the confidence of the advice. A 2025 Cochrane review of 134 trials in 20,913 adults found phosphate binders brought no clear benefit for cardiovascular death or coronary artery calcium against placebo, on mostly low certainty evidence. On dialysis, sevelamer did come out ahead of calcium-based binders on deaths from any cause.

Additives against natural phosphorus

Two foods with the same phosphorus figure are not the same problem. The phosphate added to processed meat, colas, instant noodles and ready meals is absorbed almost completely. The phosphorus locked into seeds, nuts and beans as phytate is absorbed far less. Anyone restricting phosphorus for medical reasons gets more out of reading ingredient lists for phosphate additives than out of cutting lentils.

Long-term aluminium-hydroxide antacids do the opposite and bind phosphorus in the gut, which after three months or more can drive levels too low.

The first trial that put a number on it

Unsettled. Healthy young men with normal kidneys swallowed an extra 750 mg of phosphorus a day as capsules for five days. Serum phosphate went from 0.87 to 1.00 mmol per litre, 24-hour urinary phosphate from 22.7 to 41.2 mmol, and the hormone FGF23 rose. The trialists called that a possible health risk. Five days is short, the group was small and young, and blood phosphate is a marker rather than an illness. What the trial does settle is that ordinary blood chemistry moves on a dose an ordinary processed diet can deliver.

Europe went further in 2019 and set its first safety ceiling for phosphate additives, 40 mg per kilogram of body weight a day expressed as phosphorus. The finding that came with it is the uncomfortable one. Infants, toddlers and children are estimated to pass that ceiling on average consumption, and so are adolescents whose diet is heavy in phosphates. For people over three who regularly take phosphate-containing supplements, EFSA put estimated exposure at levels it tied to kidney function.

Two cautions on reading that. The ceiling covers the additive and the 700 mg on this page covers the nutrient, so they are not comparable numbers. And note what the date admits. Before 2019 there was no group ceiling for these additives at all, after decades of use. The regulator acted, and it acted late.

American law is quieter and points the same way. Phosphorus appears once in the country's health claim rules, as a limit. A food or a day's recommended supplement intake carrying the calcium and osteoporosis claim may not contain more phosphorus than calcium by weight. No American health claim praises phosphorus for anything.

If you take phosphate supplements, one drug label is worth knowing. Cholestyramine, a bile acid binding resin, reports that oral phosphate supplements may be absorbed less well. The observation was made with another resin of the same class and carried across, which makes it a timing question rather than a warning to stop either one.
