Is grapefruit good during pregnancy?
Nobody has tested it. We found no randomized trial and no systematic review of grapefruit in pregnant women. The one question with a firm answer concerns medicines. Grapefruit juice can block a gut enzyme, CYP3A4, so more of some drugs enters the blood, and nifedipine, a drug used in pregnancy for preterm labor and for acute high blood pressure, belongs to the class most prone to that interaction.
Unsettled. Whether grapefruit itself helps or harms a pregnancy is unknown, and the reason is simple: it has never been studied on its own in pregnant women. Everything below on diet is about fruit or fruit juice in general.
Both rows are observational and come from one small cohort, so they show association and cannot show cause. Natural fruit juice showed no association and has no estimate to draw. The study reports odds ratios only, without the share of children affected in each group. Grapefruit was not reported separately. Sources: cards ev-gfpg-02 and ev-gfpg-03 below.
What the trials found
There are no trials. The closest evidence is observational and about fruit as a group.
A 2025 systematic review found that 100 percent fruit juice had a neutral association with gestational diabetes, based on 3 prospective studies. None of them looked at grapefruit alone, and the abstract does not give the number of women.
In the Spanish ECLIPSES cohort of 252 mothers and children, mothers who ate the most fruit and vegetables in pregnancy had children with lower odds of overweight at age 4, an odds ratio of 0.31 (95% CI 0.13 to 0.74). Natural fruit juice showed no association. Commercial fruit juice was linked to higher odds, 2.48 (1.06 to 5.78), a finding the authors call exploratory and which has a wide interval. These are odds from one small cohort and they cannot show that the fruit caused anything.
Grapefruit juice is a good source of vitamin C. The NIH lists it with orange and tomato juice as containing large amounts. That is true of grapefruit whether or not you are pregnant, and it is no evidence of a pregnancy benefit.
Who should be careful
The caution here is about drugs. The US FDA explains that grapefruit juice can block the action of CYP3A4 in the gut, so instead of being broken down, more of the drug enters the blood and stays in the body longer. A meta-analysis of studies in healthy adults names the dihydropyridine calcium channel blockers, the class that includes nifedipine, as the drugs most prone to this.
Nifedipine matters in pregnancy. In a Cochrane review of calcium channel blockers for preterm labor, 35 of 38 trials used nifedipine. A network meta-analysis of 74 trials in 8324 patients with acute high blood pressure in pregnancy ranked oral calcium channel blockers highest for treatment success. Neither study looked at grapefruit. They show who in pregnancy is likely to be taking the drug.
What the interaction looks like comes from a single case, a 50-year-old man with high blood pressure who was not pregnant. A single glass of 17 fl oz (500 mL) of grapefruit juice quickly raised his blood nifedipine and lowered his blood pressure for a short time. It had no effect on amlodipine, a related drug. Eating one whole grapefruit before the drug had no effect on either. One person cannot tell us that the whole fruit is safe with nifedipine.
Not for everyone. If you are pregnant and take nifedipine or any other regular medicine, ask your doctor or pharmacist whether grapefruit juice interacts with it before you drink it. That is the FDA's own advice. We found no trial of grapefruit with nifedipine in pregnant women.
What expert bodies say
We found no expert body position on grapefruit in pregnancy. The FDA's advice on grapefruit juice applies to anyone on medicines: ask your doctor or pharmacist whether it interacts with yours. The NIH Office of Dietary Supplements lists grapefruit juice among foods with large amounts of vitamin C.
How we searched
Searched: a local copy of PubMed, across all study types, for grapefruit, grapefruit juice and its active compounds with pregnancy, gestation, the placenta, lactation or nifedipine, which returned 6 records, none in pregnancy. Citrus and fruit juice with pregnancy returned 69, and nifedipine with preterm labor or hypertension in pregnancy returned 74. Europe PMC returned 21 records for grapefruit with pregnancy terms, none of them a human pregnancy study. We also searched drug interaction tables, the LactMed database, which has no grapefruit record, agency reports, the FDA site, the open web and the retraction database. Search run on 7 October 2026.
Included: one systematic review of fruit juice and gestational diabetes, one pregnancy cohort and one meta-analysis of grapefruit juice with dihydropyridine drugs. Also a Cochrane review and a network meta-analysis on nifedipine in pregnancy, one case study, two statements from the FDA consumer page and the NIH vitamin C fact sheet. None of the papers is retracted.
Excluded: two older nifedipine and grapefruit case reports that add nothing to the newer one. A grape juice study in a laboratory model of preeclampsia and a mouse study. A trial of grapefruit juice with metformin in gestational diabetes, published in a journal outside our list of trusted sources and not indexed in PubMed. Health aggregator pages.

What we could not get: any trial or review of grapefruit in pregnancy, because none exists that we could find. The full texts of the case study and of the interaction meta-analysis. The nifedipine label wording on grapefruit, which is not in our local drug label data.
What would change this answer
- Any study of grapefruit, as fruit or juice, in pregnant women with a clinical outcome. We found no ongoing trial.
- A pharmacokinetic study of grapefruit juice with nifedipine in pregnant women, whose drug handling differs from that of the healthy adults and the single man studied so far.
