Whole wheat flour: match everything else and the glucose curve is the same
In the one trial that matched wheat source, food form, particle size and viscosity, whole wheat flour gave no lower blood sugar rise than refined wheat. That was 16 healthy participants eating six medium-viscosity porridges made from one grain source and one mill, with continuous glucose monitoring and a breath test for gastric emptying. The differences in glucose response, emptying and appetite were little to none, with Bayes factors supporting the null, and the semolina porridge actually ran 966 mg min/dL below the whole wheat one. The authors of that trial left the rest of the case for whole grains standing: what they measured was the glucose curve after one porridge, not what whole grain wheat does over years. Change the food and the number moves. In 11 healthy adults of mean age 23.5 years, a cracker made with 30 percent whole wheat flour scored a glycemic index of 77, against 90 for the conventional flour control. Both of those sit in the high band, while a version with 30 percent sunflower seed flour came in at 53. US law defines this flour by what is kept: the natural constituents of the wheat stay unaltered apart from moisture, which stays at or below 15 percent.

The minerals are where the flour is least predictable. Across six analysed USDA samples, selenium ran from 0 to 68 micrograms per 100 g around a reference value of 23.6, while copper held between 0.368 and 0.652 mg and thiamin between 0.466 and 0.541 mg. A systematic review of 67 articles on post-harvest handling of biofortified crops concluded that keeping the whole grain, as whole wheat flour or as only lightly milled rice, is what preserves iron and zinc through processing. That is a conclusion about crops and handling rather than a measurement in people. Calcium from whole wheat bread is absorbed well in fractional terms and still amounts to very little. In 11 healthy women given about 80 g of labelled bread after an overnight fast, fractional absorption was 0.812 from intrinsically labelled bread against 0.792 from extrinsically labelled bread. That within-subject difference of 0.025 was not significantly different from zero, and those 80 g of bread hold only 13.3 mg of calcium. Iron added to the flour in the right compound does work. Among 401 iron-depleted children aged 6 to 15 in Indian schools, one fortified wheat-based lunch a day for 7 months carried 6 mg of iron as NaFeEDTA. Iron deficiency fell from 62 to 21 percent and iron deficiency anaemia from 18 to 9 percent, while cognitive test scores did not differ between groups. Those children were already iron depleted, so the size of that fall belongs to them and not to a reader who is not. Breeding more zinc into the wheat did nothing measurable. In 273 Indian children aged 4 to 12 eating three chapattis a day for 20 weeks, flour at 48.0 ppm zinc against 21.2 ppm in control wheat left plasma zinc flat. There was no time effect, no group difference and no interaction in the prevalence of zinc deficiency (P = 0.191). One meal can move an inflammatory marker.
