Spelt flour: wheat by law, and no gentler on a sensitive gut

Spelt is often bought as the wheat a sensitive stomach can take. In a randomized, double-blind crossover trial of 40 adults with self-reported wheat sensitivity, spelt bread caused gut symptoms that did not differ significantly from bread wheat. On sourdough bread the median change was 11.3 mm for spelt against 10.5 mm for wheat on a 100 mm scale (P = 0.144), with only 20 people in each study, so small differences could go unseen. On yeast bread, in a separate study, 5 of 20 people responded to spelt and 6 to wheat. For wheat allergy and celiac disease, US labeling law treats spelt as wheat. The FDA names spelt, Triticum spelta, among the wheat species, so spelt flour must be declared as the allergen wheat, and people with wheat allergy or celiac disease should treat it as wheat. Eight USDA samples of whole grain spelt flour averaged 14.5 g of protein and 9.3 g of fiber per 3.5 oz (100 g), with fiber running from 4.53 to 13.69 g between samples.

Minerals also swing between bags. Across the same 8 samples, zinc ran from 1.81 to 4.68 mg and magnesium from 66.2 to 162 mg per 3.5 oz (100 g), and selenium ran from 0 to 35 micrograms, since selenium in grain depends on the soil it grew in. How much of these minerals people absorb from spelt has not been studied. The blood sugar data belong to recipes. A fiber-enriched cookie made with barley flour, spelt flour and oat flakes had a glycemic index of 58.9 against white bread, or 42.0 against glucose, in 26 healthy volunteers, with lower insulin afterward. The share of spelt in it is unknown and randomization was not reported. Lab microscopy suggests protein and fiber on spelt starch granules soak up water in a dry dough and limit starch swelling during baking, a mechanism seen in vitro that may not hold for bread. In 30 healthy adults, a bread of wheat, oat and spelt flours with 22 percent dried fruit raised satiety to 6.2 against 5.2 m min for white bread, and they ate no less 4 hours later. The spelt part of that effect was never separated. In a small trial in mild to moderate Crohn disease, a 6-week diet built on spelt bread and organic red meat improved gut lesions in 3 of 4 assessable patients against 1 of 9 on a control diet (P = 0.027). Four of 18 dropped out at once, and spelt was one part of a whole restrictive diet. In 75 children with allergic disorders at one clinic, 66.6 percent had raised food IgG to spelt against 70.7 percent to wheat. Food IgG does not diagnose allergy, and there was no control group. If you have celiac disease or a wheat allergy, treat spelt flour as wheat. We found no card on spelt in pregnancy or with medicines. No health claim for spelt exists in the EU register or in US rules, and the nearest EU wording covers wheat bran and gut transit, which is bran. We found no trial or meta-analysis of spelt flour itself on absorption, portions or long-term health.
