Sourdough vs whole wheat bread: the starter and the flour are hard to separate

A systematic review of 25 randomized trials in 542 adults could not establish a clear health benefit of sourdough itself over other bread, because starter microbes, fermentation, grains and flours varied so much, and it pooled no numbers. The cleanest single test compared whole grain bread made two ways from the same recipe. In an exploratory double-blind crossover trial in 20 adults who got gut symptoms from bread, one week of the sourdough version lowered the diarrhea symptom score by 0.5 points while the baker's yeast version raised it by 1.5 (p = 0.016). The overall gut score, the main measure, did not reach significance (p = 0.051). The sourdough loaf held 0.11 g fructose per 3.5 oz (100 g) against 0.87 g in the yeast loaf, measured in one pair of breads from one bakery. Most of the blood sugar trials change two things at once. Over 6 weeks in 28 adults, whole grain wheat sourdough in place of white bread cut the glucose response to a glucose drink by 19 percent in the 14 with high glucose and insulin, a comparison that changes both the grain and the starter.

In that 6-week trial of 28 adults, fasting values did not change. In 16 healthy adults, single portions of sourdough from organic or einkorn flour gave an 8 to 12 percent lower two-hour glucose area than a commercial par-baked wheat bread, again changing flour and starter together. The EU did not authorize a claim that high-fiber sourdough rye bread lowers blood sugar after meals. That refusal concerned rye bread and means the submitted evidence fell short, which does not show the effect is absent. In a 2-month trial in 31 adults with metabolic syndrome, mean age 67, 48-hour sourdough gave no extra benefit over 2-hour sourdough. The short version lowered PAI-1 by 744 pg/mL more (95% CI 282 to 1210), in a trial that lost half its 61 starters to COVID and used one commercial brand. A medium 1.3 oz (36 g) slice of US whole wheat bread gives about 91 calories, 4.4 g protein, 2.2 g fiber and 155 mg sodium, and the US tables list no separate sourdough entry. In 5 young men, 90 percent of the lysine in whole wheat bread was available to the body, yet lysine stays the limiting amino acid because wheat holds little of it. In a 1991 isotope study in healthy women, 82 percent of the calcium in leavened whole wheat bread was absorbed against 59 percent from milk at a similar load, a share of the small amount of calcium bread contains. Bran pre-treated with enzymes and yeast, a process different from a classic starter, made phenolic acids 2 to 3 times more available in 8 men. Both breads on this page are wheat breads, and the gut symptom trial enrolled healthy adults, so it says nothing about people with celiac disease or wheat allergy. We have no card on celiac disease, wheat allergy or medicines for either bread, so the page has no safety section, and that is a gap in what we found.

Overall, it is currently difficult to establish a clear consensus with regards to the beneficial effects of sourdough per se on health when compared with other types of bread because a variety of factors, such as the microbial composition of sourdough, fermentation parameters, cereals, and flour types potentially influence the nutritional properties of bread.
