Pomegranate juice: a small blood pressure effect in a sugary glass
The best supported effect of pomegranate juice is on blood pressure, and it is modest. A meta-analysis of 8 randomized placebo-controlled trials found juice lowered systolic pressure by 4.96 mmHg (95% CI 2.25 to 7.67) and diastolic by 2.01 mmHg (95% CI 0.31 to 3.71), from small trials in mixed groups of adults. That comes in a glass of sugar. A 248 g glass of 100 percent juice carries about 134 calories and 31 g of sugars in US survey data, with 531 mg of potassium and almost no vitamin C. The inflammation story is unsettled. An updated meta-analysis of 11 trials with 696 people, largely from Iran, found C-reactive protein fell by 2.55 mg/L. An earlier one of 5 trials with 427 people found a change of 0.22 mg/L that did not reach significance, and the newer pooling showed no link to dose or duration.

The bottom line
Insulin resistance did not move. Across 15 trials with 673 people, pomegranate products did not change HOMA-IR, and that pooling mixed juice with seed oil, peel extract and peel-added bread. Drunk with white bread in 16 healthy adults, juice cut the blood glucose rise from the bread by 33 percent against a drink with the same sugar, in single meals. The sugar in the juice itself still counts. The EU refused claims that the juice supports the cardiovascular system or helps maintain cholesterol, which means the evidence did not meet the bar when it was assessed. Worries about drug interactions mostly did not hold up in controlled studies. In human volunteers, juice did not change clearance of flurbiprofen, a probe for the CYP2C9 enzyme, although it blocked that enzyme in the test tube. In 12 healthy adults, three days of 8.5 fl oz (250 mL) did not significantly change midazolam or dapoxetine levels, while grapefruit juice raised dapoxetine exposure by 60 percent. With a single 200 mg dose of cyclosporine, 17 fl oz (500 mL) of juice left blood levels within the bioequivalence range in 14 healthy men, and repeated daily juice was not tested. Two single case reports stand against that. A woman on warfarin saw her INR fall below target after she stopped drinking the juice, with no rechallenge. A man with possible underlying muscle disease on rosuvastatin and ezetimibe developed rhabdomyolysis three weeks after starting 6.8 fl oz (200 mL) twice a week, and causality is uncertain. If you take warfarin or a statin, mention the juice to whoever manages the drug. In a 4-week pilot in 24 people on hemodialysis, 3.4 fl oz (100 mL) before each session was well tolerated and did not change inflammation, oxidative stress or blood pressure. The potassium in a glass matters for anyone on dialysis. In an exploratory trial of 99 pregnancies with fetal growth restriction, 8 oz a day until delivery was linked with lower risk of infant brain injury on scans of 57 babies, with no size given in the abstract and no difference in brain volumes. Fetal heart scans showed no ductal constriction. This is one exploratory trial in a condition under medical care, and it does not establish the juice as a treatment for growth restriction.
