Pita bread: what the flatbread trials actually tested

Almost nothing on this page was measured on the pita you buy. The trials behind it fed people flatbreads rebuilt in a lab, with guar gum, konjac or chickpea flour mixed into the dough, a pita made from buckwheat, a chapatti with sprouted fenugreek. They show what a changed recipe can do, and they say nothing direct about a plain wheat pita. Even the changed recipes were hit and miss. Of ten tested in 38 adults, four cut the rise in blood sugar after the meal by 30 percent or more, and the other six did not differ significantly from an ordinary flatbread. The firmest number about pita as people really eat it concerns salt. In Lebanon white pita was the least salty bread sampled, at 1.46 g of salt per 100 g, and it still supplied the most bread salt, because people ate the most of it, about 97 g a day. Whole grain pita has a pattern in its favour. Each extra 90 g of whole grain a day, about one and a half whole grain pitas, went with a 19 percent lower risk of coronary heart disease across 7 studies. Those studies followed people and their diets, and they cannot show the bread itself did it.

If you eat pita most days, the swap the data lean towards is whole grain for white. An umbrella review of meta-analyses on diet and death from any cause puts whole grain bread at a 15 percent lower risk. That is observational data again, a pattern that nobody has tested by assigning people bread. Whole grain carries a catch for minerals. The bran holds phytic acid, a compound that binds iron and zinc so your gut takes up less of them. Leavening breaks it down. In a 1985 study of 42 adults, bran bread left to rise for 2 hours kept about 40 percent of its phytic acid and after 2 days about 15 percent, and zinc absorption rose the longer the dough fermented. In one study the review included, low-phytate white bread also raised blood iron more over two hours, 59 against 30 ug per 100 mL. Whether that turns into better iron stores is unsettled. In the longer trials it did not, and in one included sourdough rye trial ferritin, the body's iron reserve, fell from 32 to 27 ug/L over 12 weeks. The reviewers say no human study has yet asked the question with a proper control. Baking itself costs some thiamin, 20 to 30 percent of what the raw ingredients held. In Egypt the subsidised baladi flatbread is made from flour fortified with iron and folic acid, and the programme estimates this gives about 12 mg of iron and 600 ug of folic acid a day to up to 50 million people. The paper describes the likely effect on anaemia and did not measure it. Salt is the caution with a number attached. In the Lebanese survey bread alone supplied 2.86 g of salt a day, 57 percent of the WHO limit of 5 g, so if a doctor has asked you to cut salt, count the pita. A 50 g buckwheat pita, 135 kcal, left appetite and the next meal unchanged next to corn or rice snacks, so there is no sign it keeps you fuller than other starchy snacks. In 12 men, guar gum in the dough did not slow how fast the glucose was absorbed. What did fall, by 64 percent, was the glucose the liver released on its own. Several things we looked for are missing. No card here tests pita in people with diabetes, in children, in pregnancy or in anyone who has to avoid gluten, and the healthy Indian adults whose blood sugar dropped on chickpea and guar gum flatbread ate a recipe you are unlikely to find in a shop.
