Pina colada: every ingredient measured except the drink

Rum, pineapple, sugar and coconut cream have all been studied. The drink they make has not, and nothing in the corpus carries its name. What it is, before any of that, is an alcoholic drink, and the only official position this page carries is the World Health Organization's: no safe level of alcohol consumption can be named. Coconut fat is the best measured part. Across 16 trials comparing coconut oil with non-tropical vegetable oils for at least two weeks, LDL cholesterol was 10.47 mg/dL higher on coconut oil (95% CI 3.01 to 17.94) and HDL cholesterol 4.00 mg/dL higher (95% CI 2.26 to 5.73). Heterogeneity across those trials was high at 84 percent, and the fat tested was coconut oil rather than the cream poured into a glass. A second meta-analysis pooled seven randomised trials in 515 adults followed from 4 weeks to 2 years. It found HDL cholesterol 3.28 mg/dL higher (95% CI 0.66 to 5.90), body weight 0.24 kg lower (95% CI -0.83 to 0.34) and LDL cholesterol 1.67 mg/dL lower (95% CI -6.93 to 3.59). Two of those three intervals cross zero, and the review graded its own certainty very low with overall risk of bias high.

The bottom line

The sugar changes how fast the alcohol arrives, by a little. Twelve healthy people aged 19 to 64 drank 0.5 g of vodka per kilogram of body weight twice. With a diet mixer their peak breath alcohol reached 38.3 micrograms per 100 mL, against 34.8 with the sugared version. That was an open label crossover using cola rather than a blended cream cocktail. The sugared drink appeared to sit in the stomach longer, with a half-emptying time of 110.7 minutes against 100.1. The spread around both numbers was wide, at SD 66.71 and 35.42, and they were read off a nonlinear regression of breath samples rather than measured by imaging, so the ordering is not firm. Changing the mixer buys less than the label suggests. Across 14 randomised trials lasting 3 to 76 weeks, replacing sugar-sweetened beverages with artificially sweetened ones moved body weight by 0.73 kg and body fat by 0.72 percent, with inconsistent effects on glycaemic and cardiometabolic markers. The beverage definition in those trials left alcoholic drinks out altogether. An umbrella review of 20 systematic reviews of cohorts followed up to 34 years found higher intake of both kinds of drink associated with type 2 diabetes, cardiovascular disease and death. The relative risks for the artificially sweetened drinks were similar to those for the sugar-sweetened ones. That is observational and gives direction without a size, and people often move to diet drinks because they are already unwell, which the reviews cannot rule out. The alcohol is the part with an official position and no number attached. The World Health Organization stated on 4 January 2023 that no safe level of alcohol consumption can be named, and that alcohol was classified a Group 1 carcinogen by the International Agency for Research on Cancer decades ago. That is a position about alcohol in general rather than about one glass of this drink. Beyond it, this page has no card about pregnancy, none about any medicine, and none about a serving size, so it cannot tell you what a single pina colada does to you.
