Orange or sweet potato: two literatures that never met

No trial has put an orange against a sweet potato, so what follows is two separate bodies of evidence sharing one page. The orange side is juice, tested in adults in wealthy countries. Across ten randomised trials, orange juice lowered LDL cholesterol by 9.14 mg/dl (95% CI -15.79 to -2.49) and blood glucose by 2.92 mg/dl (95% CI -5.33 to -0.53), and both effects held in the subgroups drinking at least 500 ml a day. That dose is about 220 kcal of sugar every day, and body composition did not move in the same pooled analysis. When 37 centrally overweight men drank 330 ml a day for six weeks, the juice delivered as much hesperidin into the blood as a matched supplement did, and neither one changed how their arteries dilated. The sweet potato side comes from farming programmes in Africa. Growing orange-fleshed sweet potato cut the share of Ugandan children aged 6 to 35 months with inadequate vitamin A intake by more than 30 percentage points over two years, and the share with serum retinol below 1.05 micromol per litre fell by 9.5 points among the 396 children with complete data. Vitamin A deficiency was common there at the start, which is why the gain was that large.

In rural Mozambique the same crop raised daily sweet potato intake by 46 to 97 grams and vitamin A intake by 254 to 492 micrograms of retinol activity equivalents, depending on whether the group was young children, older children or women. It supplied 80 per cent of all the vitamin A the reference children took in. Intake there was measured by dietary survey rather than by blood, so the figure describes what reached the plate. A review of 25 studies found better micronutrient status only where farming households grew the crop themselves, mostly vitamin A from orange sweet potato, and reports that nothing is yet known about buying it or receiving it through schools and clinics. Those results are about programmes in places where vitamin A is short. They are not a reason for a well-fed shopper to pick one of these two foods over the other. On cooking there is direction and no numbers. A review of 67 post-harvest studies says packaging, temperature, oxygen and light govern how much beta-carotene survives storage of cooked orange sweet potato, with deep freezing at minus 80 degrees the best option tested, and all of it measured in laboratories rather than kitchens. The same review found provitamin A crops held high amounts against non-biofortified varieties of the same crop, which is a comparison with pale sweet potato and not with an orange. There is no USDA record under this pairing, so this page has no composition table and no portion size for either food. Caution here belongs to the concentrates rather than to the plate. Beta-carotene supplements did not lower death from any cause across 31 trials in 216,734 adults, at a risk ratio of 1.02 (95% CI 0.98 to 1.05). They raised lung cancer mortality by 14 per cent (RR 1.14, 95% CI 1.02 to 1.27), and the trials driving that signal gave capsule doses to smokers and asbestos workers. Nobody has shown the same for beta-carotene eaten in a sweet potato. Juice can also change how a drug is absorbed. In a review of 51 pharmacokinetic studies, grapefruit juice cut exposure to aliskiren and celiprolol by roughly 80 to 90 per cent, and orange juice appears there only as having comparable effects on certain medications, with no figure of its own and with the reviewers stating that clinical relevance remains unproven. The Moro orange extract that lowered body weight by 2.08 kg and fat mass by 1.53 kg in 252 overweight adults is a supplement, and those numbers do not transfer to a glass of juice. Beyond drugs and supplements, we found no evidence on either food for pregnancy or kidney disease, so this page cannot tell you anything about those. For children there are only the two African programmes above, run where vitamin A was already short, and nothing at all on either food for a child who is not. On labels, the European Union register lists eight beta-carotene health claims and authorises none of them, and it rejected the wording that cryptoxanthin from orange juice strengthens the immune system. Non-authorised means the evidence did not satisfy the assessment, which is not the same as a finding that the substance does nothing.
