Oatmeal and whole wheat bread: the oats have the trials
In a randomized trial of 194 adults at risk of type 2 diabetes in Germany, Norway and Sweden, 16 weeks of bread enriched with 6 g of oat beta-glucan a day made no significant difference to HbA1c against a control bread. The gap was -0.01 percent (95% CI -0.03 to 0.06). That was an oat-enriched bread, and it tells you little about a bowl of porridge. Porridge has a smaller trial of its own. In 62 Chinese adults with mildly raised cholesterol, a randomized oatmeal diet for 45 days lowered LDL cholesterol by about 13 percent against control, in a small sample whose oat dose the abstract does not give. Plain whole wheat bread has no outcome trial on this page. We found no study that set a bowl of oatmeal against slices of whole wheat bread as ordinary foods.

The bottom line
For blood sugar, how the oats are processed matters. A systematic review of 72 glycemic index tests put instant oatmeal at 75 and quick oats at 71, against 53 for large-flake and 55 for steel-cut oats, all from short tests of blood sugar after one meal. In a crossover trial of 40 people, every gram of oat beta-glucan added to instant oatmeal cut the after-meal glucose response by 7 percent and its peak by 15 percent, again a single-serving result. In 48 adults given one breakfast each way, instant oatmeal kept people fuller and they ate less at lunch than after the same calories of an oat breakfast cereal, in a single-meal study of industry-linked products. The bread does well on absorption, in very small studies. Healthy women absorbed about 82 percent of the calcium in leavened whole-wheat bread against 59 percent from milk at a similar load, in 1991 studies whose abstract gives no headcount. In 5 young men, 90 percent of the lysine in whole wheat bread was available to the body, though the bread holds so little lysine that it stays the limiting amino acid. A bread made with specially bioprocessed bran, a product changed for the study, delivered 2 to 3 times more ferulic and related acids than standard whole wheat bread in 8 men after one meal. Observational data link whole grain bread with 15 percent lower all-cause mortality (RR 0.85, 95% CI 0.82 to 0.89) and oats or oatmeal with 12 percent lower (RR 0.88, 95% CI 0.83 to 0.92). People who eat these foods may differ in other healthy ways. In two US cohorts of 122,680 people, eating cooked oatmeal or oat bran at least twice a week went with a 22 percent lower risk of gout (HR 0.78, 95% CI 0.70 to 0.86), while dark breads showed no link. Cooking oatmeal keeps about 70 percent of its folate and 80 percent of its thiamin and nearly all its minerals, by a 1975 USDA group factor for breakfast cereals. US labeling rules let rolled oats carry the soluble fiber and heart disease claim when they provide at least 4 percent beta-glucan and 10 percent total fiber by dry weight. That claim reflects FDA judgment that the evidence is sufficient and states no size of effect. The only safety card is a single case from Japan. A 52-year-old man had severe anaphylaxis from stored oatmeal that booklice had infested, and the oats themselves were not the cause. We have no card on celiac disease for either food.

Based on these results, we identified an insect body infesting oatmeal, L. bostrychophila. The serum level of L. bostrychophila-specific immunoglobulin antibodies was higher than the cutoff value; therefore, we identified the disease as anaphylaxis caused by accidental ingestion of L. bostrychophila that infested the oatmeal stored in the container.
