Oatmeal and quinoa: oats lowered HbA1c in diabetes, quinoa left weight unsettled

Across 14 controlled trials and 2 observational studies in people with type 2 diabetes, pooled from a single database search of mixed oat products, eating oats lowered HbA1c by 0.42 percentage points (95% CI -0.61 to -0.23) and fasting glucose by 7 mg/dL (0.39 mmol/L). That figure describes people who already have diabetes. Quinoa has a meta-analysis too, and the question it answers is weight. Across 7 small trials in 258 adults eating 0.53 to 1.8 oz (15 to 50 g) of quinoa a day for 28 to 180 days, quinoa did not significantly change body weight (-1.2 lb (95% CI -6.7 to 4.3; metric: -0.54 kg, -3.05 to 1.97)) or BMI against placebo. An earlier pooling of 5 small trials in 206 adults found 2.8 lb (1.26 kg) lost (95% CI -2.35 to -0.18), so the two analyses disagree and neither has enough trials to settle it. The largest single quinoa trial ran for a year in one Chinese center. Among 207 adults with impaired glucose tolerance, 3.5 oz (100 g) of quinoa a day in place of half the staple food gave lower 2-hour glucose, fasting insulin and progression to diabetes than the same amount of mixed whole grains, though the abstract reports no effect sizes. Two trials further down tested products built for the study, a biscuit made with quinoa flour and instant oatmeal with extra oat beta-glucan added, and their numbers belong to those products.

Per 100 g dry, USDA samples put rolled oats at 7.52 g of beta-glucan and quinoa flour at none, with protein close at 13.5 g against 11.9 g, though the quinoa was sampled as flour and the oats as flakes. The UK table gives raw quinoa 13.8 g protein, 7.0 g fiber and 7.80 mg iron per 3.5 oz (100 g), more iron than the 4.53 mg in the US flour samples, a gap that is part of the real spread between sources. A cup of cooked oatmeal weighs 8.5 oz (240 g) in US survey coding and a cup of cooked quinoa 170 g. Processing changes how fast oats raise blood sugar. Across 72 glycemic index tests from 20 publications and a database, steel-cut oats scored 55 and large-flake oats 53, against 71 for quick-cooking oats and 75 for instant oatmeal, all from single-meal tests whose preparation varied. In a crossover trial in 40 people, each gram of oat beta-glucan added to 27 g of instant oatmeal with milk cut the 2-hour glucose response by 7 percent and the peak rise by 15 percent, in one acute test with no quinoa arm. Making the beta-glucan thicker through processing did not change post-meal glucose in a randomized crossover trial whose size the abstract does not give. Cooked oatmeal keeps 70 percent of its folate, 80 percent of thiamin, 95 percent of iron and all of its magnesium and zinc by USDA retention factors from a 1975 table, and that table has no row for quinoa. On cholesterol, an oatmeal diet lowered LDL by 12.97 percent and total cholesterol by 8.59 percent against control in a small 45-day trial in 62 Chinese adults with mildly high cholesterol. In 40 healthy adults aged 50 to 75, a daily 15 g biscuit made with 2.1 oz (60 g) of quinoa flour per 3.5 oz (100 g) lowered LDL by 9.7 mg/dL (0.25 mmol/L) more than a wheat biscuit over 4 weeks. That is a small daily dose of quinoa inside a baked product. In 62 medication-naive patients with high cholesterol eating 2.8 oz (80 g) of oatmeal a day for 45 days, people with the TT form of the CYP7A1 variant rs3808607 lowered LDL more than G allele carriers (p = 0.02). This is a subgroup finding with few people in each genotype. US rules let rolled oats carry the soluble fiber heart health claim when they provide at least 4 percent beta-glucan by dry weight and at least 10 percent total dietary fiber. That claim records an FDA judgment that the evidence is sufficient and carries no effect size. The EU looked at a claim that quinoa favors hair growth and did not authorize it. On safety, in 12 studies of 1,066 people with celiac disease, 1.8 to 2.5 oz (50 to 70 g) of oats a day for up to 24 months kept the gut in histologic remission with negative serology. That came from a narrative review that did not address whether the oats were free of gluten contamination, and the result holds only for uncontaminated oats. One 52-year-old man in Japan had anaphylaxis 30 minutes after eating oatmeal from a container infested with booklice, a single case of a storage problem in which his skin test reacted only to that leftover oatmeal. The one finding that touches medicine sits under another heading and matters to anyone on insulin. In 15 inpatients with uncontrolled type 2 diabetes, in a tiny open-label hospital study, the insulin dose fell from 112 units on the second day to 82 and then 70 units on the two oatmeal days that followed. A change of breakfast that large is something to plan with whoever adjusts your insulin. We found no study of either food in pregnancy and none on the safety of quinoa saponins.
