Jam: the sugar swap that changed nothing, and the varnish that almost did
Jam is fruit carried by sugar, and most of the trials on it are really trials of that sugar. The firmest one is from 1991. 187 four-year-olds were randomised for two years, double blind, to sweet products made with ordinary sucrose or with invert sugar, 32 products in all, jam and marmalade among them. The swap barely touched their teeth: 3.86 decayed surfaces on sucrose against 3.10 on invert sugar (p 0.34). The larger gap was fluoride varnish, 2.86 surfaces against 4.10 without it, which did not reach significance as counted (p 0.08) and reached it only once initial lesions were left out (p 0.008). Thirty nine percent of the children did not finish the two years. Neither USDA entry in this cohort is ordinary jam. One is reduced sugar and one is sweetened with fruit juice, and their carbohydrate runs from 37.6 to 52.93 g per 100 g, with 15 mg of vitamin C and 1.5 g of fibre in the reduced sugar entry.

The bottom line
Two of the findings here are about jam as a carrier rather than jam as a food. In 30 women with metabolic syndrome on a 90-day hypocaloric diet, jam holding 3 g a day of microencapsulated fish oil lowered blood glucose, insulinaemia and HOMA against placebo, with systolic pressure also lower. Fifteen women per arm, both arms eating fewer calories, and the working ingredient was the fish oil. In 8 volunteers aged 44 to 58, calcium from a mixed breakfast that included 50 g of jam was absorbed at 17.9 percent of the dose, against 24.0 percent from milk and 28.7 percent from a calcium tablet. Only the milk against meal gap reached significance, and jam was one item in that breakfast, so what was measured is a breakfast. The most quotable number about jam is also the softest. In 27,786 Swedish adults followed a median 25 years, those eating more than 10 servings of jam or marmalade a week had all-cause dementia at 0.86 times the rate of those eating under half a serving (95% CI 0.77 to 0.97). That is an association from one cohort, people who eat that much jam differ in many other ways, the paper puts no gram weight on a serving, and total free sugar intake showed no link to dementia at all. No trial has fed anyone jam and counted dementia. The label is worth reading rather than trusting. Across 4,002 packaged foods sold in Greece between September 2020 and January 2021, jams carrying a natural or organic claim held more energy and more total sugar than the conventional jams they were matched against. That is composition printed on packs in one country over five months, and it measured nothing in anyone who ate them. Low sugar jam is a manufacturing problem before it is a nutrition one. Apricot, guava and strawberry jams rebuilt with xylitol blends in place of sucrose needed 0.2 g of added calcium chloride to hold their texture, and storage at 4 degrees C kept microbial load under 20 cells per gram. Plasma glucose fell in the lean and obese patients with diabetes who then ate those foods (p below 0.05), but the foods were a set of jams, jellies, cakes and biscuits rather than jam by itself, and the 1997 abstract gives no participant count. The only caution card on this page is that caries trial in four-year-olds, and what it showed is that changing which sugar the jam was made with did not change the count. We have no card on how much sugar ordinary jam holds, none on any upper limit for free sugar, and none on jam in pregnancy, alongside medicines, or for oxalates and stones. Read that as evidence we did not find rather than as reassurance.

The attainment of a suitable texture may be more difficult in xylitol and sorbitol jams, therefore 0.2 g CaCl2. 2H2O was added. Storage of these jams at 4 degrees C improved their keeping quality significantly (p < 0.05), where the microbial load was less than 20 cells per gram and the products were free from molds and yeasts.
