Cake: after dinner it swings glucose more, and baked does not mean allergy-safe

A slice of cake is the same slice whenever you eat it, and the body does not treat it that way. In 17 healthy women aged about 21, with a BMI of 20.7, a 498 kcal slice of baked cake eaten after dinner raised the day's glucose swings to 3.54 mmol/L, against 2.73 mmol/L when the same cake was eaten mid-afternoon. They wore a continuous glucose monitor for 7 days. That is one day's swing in lean young women without diabetes, and it is the only trial on this page that fed people cake as cake. The rest is about cake as a member of a group. Across 17 prospective cohorts with more than 875 000 adults, refined grain intake was not associated with cardiovascular disease, with a hazard ratio of 1.08 (95 percent CI 0.99 to 1.18) and heterogeneity of 70 percent. That held in the analyses that pooled cakes, cookies and pastries with bread. What the word cake hides is the range. Across 44 USDA records, fat runs from 0.3 to 23.3 g per 100 g and saturated fat from 0.04 to 9.9 g, from angel food to frosted chocolate cake. The portion hides as much. US intake surveys record a slice of sponge cake at 65 g, a cupcake at 90 g and a slice of cream cake at 200 g, and those per-portion values are recalculated from 100 g rather than weighed.

In 89 children aged 8 to 10, doubling the portion of loaf cake and other foods made bites 13 percent larger and cut chewing cycles per gram by 8 percent, whatever the child's weight status. That was one laboratory eating session that measured chewing rather than intake. In a cluster randomised trial across 11 English secondary schools, labelling cakes and sweet biscuits with the exercise needed to burn them cut purchases by about 11 items per week per 100 students (adjusted mean difference 0.112, 95 percent CI 0.045 to 0.179). That ran six weeks, counted purchases rather than what was eaten, and 7 of the 18 randomised schools did not take part. One narrow finding runs the other way. In 12 healthy adults, vitamin D3 baked into a sponge cake gave a 26.6 percent larger postprandial chylomicron response than the same dose in a biscuit (p = 0.047). The model foods were made to identical composition, and lutein showed no difference. Baking forms acrylamide, and what that means for people is open. A meta-analysis of 28 laboratory occurrence studies of wheat-based foods put pooled acrylamide at 136.29 micrograms per kg, the highest of three processing contaminants, with most samples being bread, cookies and pasta rather than cake. In 80 597 French women followed a mean 8.8 years, cakes and pastries were among the main sources of dietary acrylamide. The top intake quartile had a breast cancer hazard ratio of 1.21 (95 percent CI 1.00 to 1.47), a borderline figure, and 1.40 (1.04 to 1.88) for premenopausal cancer, from self-reported diet and composition tables. In 120 852 Dutch adults aged 55 to 69 followed 13.3 years, each extra 10 micrograms a day was not associated with colorectal, gastric, pancreatic or oesophageal cancer, with a colorectal hazard ratio of 1.00 (0.96 to 1.06). Two cohorts, two answers. The EU acrylamide regulation sets benchmark levels for biscuits, wafers, scones and gingerbread and never mentions cake, which says nothing about how much is in one. The cautions with numbers are about children with egg or milk allergy, and baked is not the same as safe. Of 236 egg-allergic children challenged in a clinic with one baked muffin holding 1 g of egg protein, 64 percent tolerated it and 36 percent reacted. Twelve of the 86 who reacted had anaphylaxis, four of them at under 100 mg of heated egg protein. Among 50 milk-allergic children under 6 selected for high risk, 66 percent with prior anaphylaxis, 36 percent reacted to a supervised baked milk challenge, 39 percent of those reactions were anaphylaxis, and the median threshold was 138 mg of milk protein. The same baked form is also how tolerance was built in one trial, and only in children who had first passed a supervised baked-milk challenge in a clinic. In 84 milk-allergic children aged 6 months to 3 years selected that way, eating it as a muffin for 6 months and then as baked cheese for 6 months brought 88.1 percent to tolerate unheated milk at 1 year. Strict avoidance gave 66.7 percent, in an open single-centre trial. In 27 observational studies of 30 805 preschool children in sub-Saharan Africa, sweet foods of all kinds were linked to 2.55 times the odds of overweight or obesity (95 percent CI 1.86 to 3.48), odds rather than risk, with heterogeneity of 99.4 percent. We have no card on cake in pregnancy or with any medicine, and none on wheat, nuts or any allergen other than egg and milk, though a cake may carry all of them.
