Brown rice or jasmine rice: closer on blood sugar than the labels suggest

In a meta-analysis of 7 trials in 417 adults with prediabetes or type 2 diabetes, eating brown rice instead of white rice did not change HbA1c (p = 0.15) or fasting glucose (p = 0.95). The evidence is of low to moderate quality, from short trials, and some of them were not randomized. Brown rice did lower body weight in those trials, by 4.9 lb (95% CI 2.8 to 6.9; metric: 2.2 kg, 1.26 to 3.13). Jasmine rice is a fragrant white rice, and only one study has tested it against brown rice. On a single test meal it measured a glycemic index of 80 for jasmine and 78 for brown in Chinese adults, and 68 and 65 in Europeans. That study did not compare the two rices statistically, so treat them as close. A systematic review of rice studies found that brown rice gives a lower blood sugar response than white only when both are cooked for the same time. At the longer cooking times brown rice usually gets, the gap became small and inconsistent.

Where brown rice clearly differs is the bran. UK composition data for boiled rice give brown rice 1.5 g fiber, 48 mg magnesium and 0.53 mg iron per 3.5 oz (100 g), against 0.5 g, 5 mg and 0.04 mg for white long-grain rice. Jasmine has no record of its own, so white long grain is the nearest stand-in. Energy is the same, 132 against 131 calories. A cup of cooked brown rice weighs 6.9 oz (195 g) in US survey data. Short trials show a smaller effect than the reputation. In a crossover test on 10 healthy and 9 diabetic volunteers, brown rice had a 12.1% lower glycemic index than milled rice of the same variety in the healthy group and 35.6% lower in the diabetic group, from single meals. In 31 New Zealand adults with type 2 diabetes, less-processed whole grains lowered glucose after breakfast by 9% (95% CI 3 to 15) over 2 weeks, but brown rice was one of three grains and its share was not split out. In a 3-month trial in 166 overweight adults in Chennai, parboiled brown rice did not beat white rice overall. In the subgroup with metabolic syndrome it lowered HbA1c by 0.18 points, with a third of participants dropping out. Observational data point the same way and cannot show cause. A meta-analysis of 16 cohorts links brown rice with lower type 2 diabetes risk and white rice with higher risk, and the authors note the rice results rest on few studies. Pooled US cohorts of 207,556 adults followed up to 26 years found no link between cardiovascular disease and either rice at 5 or more servings a week (HR 1.01 for brown, 0.98 for white). The EU refused two claims for germinated brown rice, one on blood glucose and one on weight loss. Refusal means the evidence fell short at assessment. The cautions are arsenic and, rarely, allergy. In laboratory tests, cooking brown rice in excess water cut its inorganic arsenic by about 50% and polished long-grain rice by 40%, and jasmine was not reported separately. For infant rice cereals the FDA sets an action level of 100 ppb inorganic arsenic and notes that white rice cereals tend to carry less than brown ones. That applies to baby cereal, not to rice on an adult plate. A case report describes a 14-year-old in Japan with exercise-induced anaphylaxis after polished rice whose skin test was also positive to rice bran. Nobody has compared brown and jasmine rice over weeks of eating.
