Beef liver: the vitamin A is both the point and the limit
Start with contamination, because it is the one thing here counted at scale. Ochratoxin A, a mould toxin, showed up in 2 percent of cow liver samples against 39 percent of meat and offal overall, in a meta-analysis of 8,585 mostly European surveillance samples that counted detections rather than amounts. That was the lowest share of any food type in the analysis. The rest of the page turns on one nutrient. The NIH Office of Dietary Supplements puts liver at the top of the preformed vitamin A list, alongside fish, eggs and dairy. The same fact sheet says the body takes up 75 to 100 percent of the retinol in food and usually 10 to 30 percent of beta-carotene, ranges given for the nutrient rather than measured on a cooked portion of liver. The USDA record behind it puts raw beef liver at 4,968 micrograms RAE per 100 g, with five records spreading up to 28,319, so cuts and countries differ more than foods do. A single 100 g portion is already past the 3,000 micrograms RAE a day that experts advise women who are or might be pregnant to stay under, a ceiling written for supplements rather than for dinner.

The bottom line
Raw liver is the part people argue about. Japan banned it in restaurants in 2012, and weekly asymptomatic E. coli infections went from 0.45 to 0.51 per million people, a trend change of 0.002 per million per week, 95 percent confidence interval -0.03 to 0.04, P = 0.93. This was tested, and the ban did not move the national count. It does not follow that raw liver is safe: people kept eating it at home, and a national restaurant rule is a blunt instrument. In a Canadian survey of 316 samples from Ottawa, Campylobacter jejuni was found in 4, one of which was raw beef liver, a single observation that gives no rate for liver on its own. Two Iranian studies count liver against health outcomes, and both are observational. Among 242 people with confirmed lung cancer and 484 controls matched on age, sex and residence in Tehran, liver stayed a risk factor after adjustment at P = 0.004, with no odds ratio reported and 66.5 percent of cases smoking against 40.3 percent of controls. Among 482 Tehran women aged 20 to 50, the top quarter for red meat had 2.51 times the odds of depressive symptoms against the bottom quarter, 95 percent confidence interval 1.32 to 4.76, with liver folded into one combined red meat score. Those are odds from a single time point, and low mood changing what people eat fits the data as well as the other direction. Where liver looks useful, the studies are small. Among 150 girls aged 11 to 19 near Islamabad, 77 percent of them anaemic, beef and liver was the only food group whose intake tracked with haemoglobin, with no effect size reported and no adjustment described. In 96 Indonesian children aged 12 to 23 months, meals fortified with powdered beef liver scored 3.3 to 3.5 out of 5 for liking after two weeks at home, and much of the powder went uneaten because of its smell. That trial measured whether toddlers would eat it, not what it did to them. The caution worth your attention is the vitamin A itself. It is fat soluble, so the body stores the excess in the liver instead of flushing it out, and repeated portions accumulate in a way that water soluble nutrients do not. Acute poisoning follows one or a few doses above roughly 100 times the RDA, a level the fact sheet describes for supplements and one that liver would reach only in a very large portion. In a South African cohort of 94 children aged 3 to 5, higher stored vitamin A went with lower height and weight scores and explained 6 to 10 percent of the variation before a supplement dose. Supplements, fortified food and liver all fed the same excess in that cohort, and there was no group of normal status to compare against, so liver cannot be pulled out of it. If you are pregnant, might be, or are feeding a small child, how big a portion and how often is a question for the person treating you, because nothing in our sources tests how often liver can be eaten in pregnancy. The same fact sheet names retinoid medicines, isotretinoin for severe acne and etretinate for severe psoriasis, alongside intakes above the upper limit as causes of those same birth defects. We also found nothing that measures what frying or braising does to the vitamin A in liver.
