Applesauce: mostly studied as a way to take pills

The research on applesauce is mostly pharmacology. It appears trial after trial as the soft food a capsule gets emptied onto for someone who cannot swallow it, and the answer changes with the drug. In 48 healthy subjects, nilotinib stirred into one teaspoon of applesauce matched the intact capsule, with a geometric mean ratio of 0.99 for total exposure (90% CI 0.94 to 1.04). Viloxazine extended-release on one tablespoon gave 90.10 percent of the peak concentration and 93.71 percent of the exposure of a whole capsule in 25 healthy adults, both inside the 80 to 125 percent band that counts as no difference. Apalutamide went the other way in 12 healthy men, with peak exposure 27.6 percent higher than whole tablets. Valbenazine granules were chemically stable, with 95.6 to 100.2 percent recovered across applesauce, pudding and yogurt after two hours, and sprinkling them on applesauce still gave a lower peak and a lower total exposure than the reference capsule. For dordaviprone total exposure stayed inside the limits while the lower bound for peak concentration fell to 0.70. Every one of these was a single dose in healthy volunteers, tested for that one drug, which is why the pharmacist and not the internet decides whether your capsule may be opened.

What is in the jar is not one number. Across eight analysed samples of a single unsweetened product, malic acid ran from 281.0 to 644.5 mg per 100 g, iron from zero to 0.14 mg and fructose from 5.48 to 7.01 g. Canning is the reason the standard unsweetened record carries added vitamin C: the USDA retention table gives canned fruit 50 percent of its vitamin C and 50 percent of its folate, while iron, magnesium, phosphorus and zinc survive in full. That factor is set for the whole group of fruits and juices at that method rather than for applesauce by name. Release 6 of the table has no carbohydrate row at all, so the sugar in a sweetened jar is a question for its label. In one trial of 15 women, net acid excretion fell after both fresh apple and applesauce, where it rose on the control treatment (P = 0.041). The rise in urinary calcium was smaller after both (P = 0.024). The applesauce serving was 2.5 times the size of the fresh-apple serving, and the trial did not test whether the two were equivalent. The measurement was urine over four and a half hours on one morning, and bone was never looked at. Portions drift upward quietly. A single-serve pouch is 90 g in the USDA tables and a snack cup 113 g, about a quarter more food. Asking children aged 7 to 11 to imagine how food would taste made them choose 7.1 percent less brownie and changed nothing about how much applesauce they took, in a trial of 171 children that recorded what they selected rather than what they swallowed. Who should be careful. If a capsule is going onto the applesauce, that is a question for the prescriber or the pharmacist and not for this page. Every number above was a single dose in healthy volunteers, tested for one named drug, and the shift ran in both directions depending on which drug it was. The other event on this page is lead. Cinnamon applesauce pouches poisoned children in 2023 and 2024, with 519 cases reported to CDC from 44 states, Washington D.C. and Puerto Rico as of 22 March 2024, and 2.18 ppm of lead in the agency sample of one apple cinnamon puree, more than 200 times the 0.01 ppm action level the FDA had proposed. The lead came with the cinnamon added to the puree, and the agency called economically motivated adulteration its leading hypothesis, so the warning is about one adulterated ingredient rather than about apples. If someone at home has trouble swallowing, the spoon is the lever. Across 343 self-selected spoonfuls of puree taken by 49 outpatient adults the median bite was 10.3 to 11.1 mL, and larger bites went with more residue in the throat, most of all in people with severe to profound impairment. In 37 such patients, applesauce left less residue in the upper throat than pudding of the same 5 mL volume, and dyeing the applesauce blue or green made the residue ratings worse, with no difference in penetration or aspiration in that comparison. None of this gives a safe spoonful for one person: bite size was not predicted by age, sex or the severity of the swallowing impairment, and that null result is the honest end of the evidence. One label note. Applesauce is the example the United States regulator uses in its own rule for a food that is cholesterol free without anything being done to it. Such a label has to disclose that cholesterol is not usually present in the food rather than sell that as an achievement.
